Tattoo Management Studio Express Edition has the ability to print pre-filled consent (release) and aftercare forms for services. For each Service Type you can specify the following forms.
Consent Form: The consent form generated for a customer meeting the minimum age requirements of a service.
Guardian Consent Form: The consent form generated for a customer that does not meet the minimum age requirements of a service. If no form is specified, the service will not be able to be saved if the selected client is a minor.
Aftercare Form: The aftercare instructions generated for the specific service type.
Tattoo Management Studio Express Edition comes with pre-defined consent and release forms provided that fit most state laws, but in some cases you may wish to customize these forms. Forms are completely customizable and allow you to format them as necessary.
IMPORTANT: The forms that come pre-installed with Tattoo Management Studio Express Edition are updated each time you install a software update. Meaning, if you make a change to our default forms, your changes will be overwritten if you install a new version. If you wish to preserve your changes between software updates, you must create your own forms, rather than modifying the defaults provided. If you would like to make a simple change to our forms or use them as a starting point, you may use the Clone Action to make a copy of it, and modify from there, which will be demonstrated below.
Now that you've read the above paragraph labeled IMPORTANT (you did read it, right? If not, read it now.) we can proceed. Let's modify the Tattoo Consent form to include the clients mobile phone number as well as bold some important text we want to make sure the client reads carefully.

In the Application Window, click the Reports, Forms & Letters
Navigation Group, and then click the Forms & Letters
Navigation Item. This will bring up the Letter List View.

Locate the Tattoo Consent Form by REV23 Development, select it and click the "Clone" action on the ribbon. This will make a copy of this form that is safe to edit and open it in the Letter Detail View.

Change the Name property to a name that suits your needs, and change the Author.

Click the "Edit Form/Letter" Action on the Ribbon to invoke the Letter Editor.

The Letter Editor is similar to most word processing applications. You can format text, change margins & page orientation, etc... Using these features is outside the scope of the User Guide.

The Letter Editor is just like using most word processing applications. You can select a block of text, and click the "Bold" button in the Font group to bold text, etc.. Lets highlight the first paragraph and make it bold. Use your mouse cursor and click the beginning of the paragraph, and while holding the mouse button, drag to the end of the paragraph.
The paragraph is selected.

Now, in the Font Group on the Ribbon, click the "Bold" button

(or press Ctrl+B on your keyboard.) The paragraph should now be formatted similar to the image below.
The paragraph is now bold. You can also change the font size, underlining, obliqueness, font color, etc... while the text is highlighted.

Now let's add the Mobile Phone Number underneath the Home Phone Number. Place your cursor after the Home Phone Number field and hit ENTER on your keyboard to go to the next line. Type in "Mobile Phone:" and press the TAB key.

Tattoo Management Studio Express Edition uses Merge Fields to pre-fill information on Forms & Letters. These fields tell Tattoo Management Studio Express Edition which field to read from the database when the form is generated. We will insert the customer's mobile phone number in this space. On the Ribbon click the "Mailings" tab, and click the "Insert Merge Field" action to show a list of available fields

You can see the list of fields we have availble to merge. Fields with an arrow to the left of them indicate there are additional fields belonging to that object. Click the arrow next to Customer.
Customer has additional properties that can be merged such as Age, Birthday, First Name, Last Name, etc...

Scroll down and find Mobile Phone Number in the Customer field list. Highlight it and click the "Add" button.

The Mobile Phone Number merge field will be added in the spot our cursor was. If done correctly, your form should look similar to the image below.
The form now includes the customer's mobile phone number that will be pre-filled when the form is generated.

Click the Save button on the Letter Editor's Ribbon.

Click the Save button on the Letter Detail View to Save our changes and return to the Letter List View. Our new tattoo consent form will now appear in the list.
The Letter List View will now contain the new form we've created It will have the name and author you specified in Step 3.

Now, we need to instruct Tattoo Management Studio Express Edition to use this consent form for the Tattoo service type. Open the Tattoo Service Type Detail View (see
Configuring Service Types) and drop down the Consent Form property editor. Our new tattoo consent form will appear in this list, select it and Save the Tattoo Service Type.
The Tattoo Service Type's Consent Form property is now set to our new consent form.
We're all done! Tattoo Management Studio Express Edition will now use our new consent form anytime a Tattoo service consent form is generated.
